Information for parents/guardians
If you child's coach has registered to take part in the CoachMADE program, you child will be asked to complete a short survey on three occasions between February 2017 and August 2017.
The questions in the survey focus on the children’s demographics (age, gender, ethnicity, sport history, etc.) the motivational strategies that their coach uses within the context of their team, and your child’s perspective, attitudes and susceptibility to using performance enhancing substances.
Your child is free to refuse to answer any questions; there are no right or wrong answers, and all answers will be kept strictly confidential.
The completed survey will not be shared with anyone such as their coach and teammates.
The survey will take approximately 15 minutes of your child’s time and will be carried out at the beginning or end of a training session, as agreed with the coach. A member of the research team will be present and available to answer any queries.

If you DO NOT wish your child to take part in the project you must complete the form at the end of the information sheet and return it to the project team by Friday 17 February.
